Root Cause Failure Analysis ... the role of NDT

The term Root Cause Failure Analysis (RCFA) has been associated with equipment reliability for the past twenty years or so and is generally understood to mean find and fix the causes of equipment failure. Many companies have no structured failure analysis. Instead, they operate on the breakdown maintenance system: If something breaks, they fix it, but they don’t ask why it failed. When the problem happens a few more times, they may look for a root cause and fix it differently. These organizations often have measuring systems that record how long it took to repair a failure. Maintenance personnel get very good at repairing the equipment, but the failures keep happening. Organizations that are striving to learn and improve often have some formal root cause analysis program but the effectiveness is often limited and repeat failures are not uncommon. Unfortunately, in most businesses today, the majority of failures will be repeated and they will be repeated simply because the employees have never learned effective problem solving skills.

This talk will present a simple evidence based problem solving methodology on how one can effectively determine Root Cause(s) that lead to the occurrence of an incident. The role of Non- Destructive Testing techniques in successful application of this methodology will be highlighted through some case studies.

About Speaker

Dr. Lim is the Director of Safety Accident and Failure Experts Limited. Graduated with a Mechanical Engineering background, supplemented with research in Materials Engineering, Ir Dr Lim has over 15 years of experience in conducting failure investigations. He is currently a corporate member of Hong Kong Institution of Engineers, and also a member of the Academy of Experts.

He has investigated a wide range of technical problems covering all aspects of engineering. Up to date, Ir Dr Lim has completed more than 1500 technical reports, and has been requested on more than 50 occasions to act as an Expert Witness in Court. Ir Dr Lim is also a technical assessor assisting the Hong Kong Accreditation Services of HKSAR Government to accredit laboratories conducting Non-Destructive Testing in the construction industry under HOKLAS scheme.

Guided wave Inspection - The Future

Plant Integrity Limited has developed a permanently installed tool-head called Teletest Perm-A-MountTM. This device consists of a transducer array attached directly to the pipe surface providing all the benefits of Teletest Focus Guided Wave Inspection without the problems with Access. Perm-A-Mount is a low-cost, long-life tool installation intended for regular monitoring of piping systems in environmentally hostile, safety critical or difficult to access areas.

Typical Applications for this technique include:

  • Road or River Crossings
  • Elevated lines
  • Insulated lines
  • Off shore risers
  • Buried lines
  • Sub sea lines

The installation process has been developed in conjunction with TWI Ltd’s surfacing and corrosion departments and tested to ensure that the tool will not be a future source of corrosion in piping systems.

The tool head is connected to existing Teletest Focus Units via an external communication box that can be located up to 25metres from the tool head. The data collection is very simple to execute and can be either be analysed on site by a CSWIP trained operator or remotely.

  • Automated 3 mouse-buttons click data collection. Provides the flexibility for on-site analysis or remote analysis by CSWIP Trained operators
  • Low Cost
  • Multimode Wave Form: Transducer arrays are optimised for pipe diameter, wall thickness & contents, maximising detection capabilities
  • Excellent Data Stability & Repeatability: Data collected from Perm-a-mount tool heads is highly repeatable allowing very small changes in pipe condition to be detected
  • Wide Temperature Range: Perm-A-Mount can operate across a wide range of temperatures from -40°C to +125°C
  • Pipe Size Range: ASME Schedule 2” to 36”
  • Unique Serial Numbered Tool Heads
  • Improved detectability - Overlay of scans allows improved analysis and earlier detection of defects
  • Low Profile: The tool is only 15mm thick & 150mm long
  • Mechanically Robust Construction - Fatigue testing showed no degradation of tool after 4.1 million cycles
  • Environmentally Robust - No degradation of signal after salt spray testing

About Speaker

Mr. Ian Daniel has over 25 years O&G inspection experience. Ian has been involved in welding inspection, mechanical testing, NDT, R&D of advanced NDT, RBI, Integrity Management and contract management with companies such as Unit Inspection (1984 to 1992), RTD (1992 TO 1998) and Lloyd's Register (1998 to 2003). Ian has held a management role in TWI since 2004 and is the Sales and Operations Manager for TWI's wholly owned subsidiary company, Plant Integrity Limited based in Cambridge, UK. Plant Integrity Limited services include the development and manufacture of the guidedwave product called Teletest Focus and also support the industry with services that encompass RBI, NDT, Condition Monitoring, Assessment and Repair of O&G assets.

29th Technical Talk – Application of NDT in Building Diagnostics – Sharing of Building Inspection experience in Hong Kong

Synopsis

Recently a 50 years old building collapsed in an urban area of Hong Kong. Four occupants were killed and a number of them injured. This incident raised an alert of the city over the Hong Kong urban area. The government will enact a law for Mandatory Building Inspection Scheme (MBIS) for old buildings of 30 years old and the implementation of it will be end of the year. A registry of Registered Inspector (RI) will be kept by HK Building Authority to liaise with building owners and carry out the building inspection. The RI shall be RPE of HKIE. A profession named as Building Diagnosticians will emerge to actually conduct the inspection. HKAS will offer accreditation of building diagnostic testing. HKCI will publish a Code of Practice for Condition Assessment for Old Buildings. Ir. Cheung will talk about this implementation of MBIS from the perspectives of HKIE, HKAS and HKCI.

About the Speaker

Ir. CK Cheung is a Senior Accreditation Officer of the Hong Kong Accreditation Service (HKAS). He is a Registered Professional Engineer (RPE) in Civil and Geotechnical Disciplines of the Hong Kong Institution of Engineers (HKIE). He received a MSc and DIC from Imperial College of London and a MEng from Hong Kong University (HKU). He is the President of Hong Kong Institution of Certified Auditors (HKICA). He is also the Vice-President of the Hong Kong Concrete Institute (HKCI). He is also both a Lead Evaluator of the Asian Pacific Laboratory Accreditation Cooperation (APLAC) and Pacific Accreditation Cooperation (PAC). Mr. Cheung now is leading a six members APLAC and PAC team to perform a peer evaluation of Singapore Accreditation Council (SAC) here in Singapore during this week.
